1 Ramsay Bolton - Game of Thrones
Ramsay Bolton, also known as Ramsay Snow, is a fictional character in the A Song of Ice and Fire series of fantasy novels by American author George R. R. Martin, and its television adaptation Game of Thrones.
He's horrible, he killed his stepmother and his little brother among others and he tortured and castrated Theon Greyjoy.
2 Ivar the Boneless - Vikings
Ivar is a character that lacks empathy and who at times is very sadistic. He killed his own brother out of uncontrolled rage, he let his new born child die in the woods, he killed his wife, he burned people alive, he rules like a tyrant and he likes to emotionally manipulate others.
I did some research on the real Ivar the Boneless (that actually existed) and apparently he was even worse than his depiction in Vikings.
Haha what a name.
3 Nurse Ratched - One Flew Over the Cuckoo's Nest
If you've seen the movie, you know how horrible she is. She manipulates the people she is supposed to take care of and treats them horribly, she even caused one of them to commit suicide. The ending of the movie of course is the worst thing she did (which I won't spoil just in case).
I don't consider her sadistic at all. She's a cold, emotionless, unbending instrument of the unempathetic 'system'. Now is she an awful, manipulative person? Oh lord, yes. Total cow. But sadism? Nah, that would involve feelings and she doesn't have any of those.
4 Carnage - Marvel Comics
Carnage (Cletus Kassidy) is a fictional supervillain appearing in American comic books published by Marvel Comics, usually as an enemy of Spider-Man. He appeared for the first time in The Amazing Spider-Man #361.
Carnage is what happens when you mix a murderous psychopath and an alien symbiote. Before becoming Carnage, Cletus Kasady was a serial killer who took pleasure in butchering his victims and left his mark with their blood.
He's a psycho serial killer who makes fun of killing everyone and everything. Moreover, he has a simbiote, and is even one of the strongest simbiotes ever existed. In my opinion, he should be first with ramsay being on 2nd place
He's not only sadistic, but he's also really scary, probably the scariest Marvel villain.
5 Joker - Batman
The Joker is a fictional super villain created by Bill Finger, Bob Kane, and Jerry Robinson who first appeared in the debut issue of the comic book Batman (April 25, 1940) published by DC Comics. Credit for the Joker's creation is disputed; Kane and Robinson claimed responsibility for the Joker's design, while acknowledging Finger's writing contribution. From the moment he killed Gabriel, I wanted to see him dead the whole movie and I couldn't stand seeing him. That's just shows how good this actor is.
8 Trevor Philips - Grand Theft Auto V
Trevor Phillips is a character from the game Grand Theft Auto Five (GTAV), a Mature rated game from 2013, created by Rockstar. He is one of the games 3 antagonists. He is the former friend of Michael DeSanta, who he was part of a bank heist with 9 years prior to the main plot. He is known for being a psychotic, racist, blood-thirsty gang leader of other crazy characters in GTAV.
The most insane, violent, twisted and sadistic of the trio of main characters in GTA V. He goes on rampages, he manipulates other characters, he tortured another and he feels no remorse for his actions. He is capable of love though which I guess is a redeeming quality.
Trevor is a psychopath and a sadist, but he's still my favourite GTA V protagonist.

He's indeed a cold racist plantation owner. I see no humanity in this man.
