It's been over a year since I watched Your Lie in April, but it still makes me cry just thinking about the ending. I knew what was coming for Kaori, but I still hoped that somehow she would survive and that she and Kousei would end up together. I refuse to believe that Kaori died. I've even written an alternate ending where Kaori survives the surgery. I grew to love Kaori so much that when she died, it felt like losing someone in real life. Your Lie in April was the best anime I've ever seen, and without a doubt, the saddest.
These two loved each other, but Julia was with Vicious. They decided to run away together as star-crossed lovers, but Vicious got the better of them. It was so sad seeing all the effort they put into being together completely destroyed.
Yui spent her life confined to a wheelchair, with her mother having to do everything for her. In purgatory, she was free to do whatever she wanted, but the one thing she desired most was to get married, and Hinata promised to marry her. Sadly, Yui fulfilled her final wish in purgatory and had no reason to stay, leaving Hinata behind.
Sometimes, just to torture myself, I look up that really sad scene from episode 10 and cry my eyes out.
Hige and Blue were such a cute couple. When they reunited, you could tell how happy they were to see each other. But in the end, it wasn't meant to be. Blue and Hige die side by side together in the finale of the anime.
These two wanted to be married, but a horrible incident scarred Kana for life. So Hatori removed her memories and now watches Kana from afar instead.
Lelouch unintentionally uses his Geass powers to possess Euphemia, causing her to order her people to kill the Japanese in a massacre. Lelouch is then forced to kill her. Suzaku does everything in his power to try and save her, but she dies.
Damn you, Creed! Train and Saya were heading in the right direction together, but you had to ruin it all!
Lucy's life has been nothing but a nightmare. She grew up in an orphanage, was bullied, and the first person who ever trusted her betrayed her. She even saw the puppy she loved die, and Kouta fell in love with his cousin. Kouta was the only person who ever saw any good in her and didn't want her dead just because she was a Diclonius. But they couldn't be together.
This is why Yuka isn't loved in the Elfen Lied community. Kouta and Lucy should have been together!
Gainax endings can be the worst. Simon and Nia were just about to get married, and then she disappears...
These two were just starting a family with a daughter named Yui and a son named Ame. They were a happy couple despite the fact that one of them could change into a wolf. But Wolf Man died trying to feed his mate and children, leaving a heartbroken Hana to figure out how to raise Wolf Children.
The Newcomers
The reason I believe they are one of the saddest couples is that Zeref killed Mavis because he loved her so much. Due to their shared curse, his love for her ultimately led to her death. Zeref spent 300 years wanting to die, and when he finally met someone he loved, he thought they could at least be together. Tragically, he ended up killing her. In the end, even Mavis couldn't hate him because they loved each other so deeply. It's heartbreaking because he killed her simply because he loved her.
It's so sad! Maes loved his wife more than anything, well, maybe except for his daughter. And then... Oh gosh. I cried so much.

Truly the saddest anime couple. Gin left Soul Society to spy on Aizen, the top villain. He wanted to become a Shinigami to protect Rangiku. It was heartbreaking when Rangiku saw Gin lying dead on the ground after being killed by Aizen. She rushed towards Gin and cried.
Rin was everything to Obito, and she was killed by his most trusted friend.
This isn't canon (yet), but their story is just so tragic. Please make them canon already - Mitsuba has already suffered enough.
Such a beautiful and yet incredibly sad couple. Spoiler Alert: Since they had to return Kurisu to the time when she died, Okabe boldly tells her that he loves her, and the two share what I think is anime's most heartfelt kiss. It's a romantic and depressing pair, saying goodbye to each other for good (or maybe not for good) by kissing on the lips at least four times. Who cares? They kissed!
These two are so emotionally beautiful. They had to send Kurisu back to the day she was killed, and to say goodbye, they ended up having multiple "goodbye, I love you" kisses, making it anime's most heartfelt kiss.
